Position Job Title: Physical Therapist Profession: Physical
Therapist Specialty: Occupational Therapist Duration: 13 weeks
Shift: 5x8 Hours per Shift: 8 Experience: Minimum of one year in
Long Term Care/Home Care setting License: Current license in
Physical Therapy Certifications: BLS (AHA ONLY) Must-Have: Oasis
experience Description: Provides case management and direct patient
care within a team environment. Administers comprehensive physical
therapy modalities in the home setting. Assists in evaluating the
level of function and helps develop the plan of care. Prepares
clinical and progress notes and advises and consults with the
family and other personnel. Participates in in-service programs.
Promotes effective working relationships and works as part of the
team to meet goals and objectives. Utilizes resources, supplies,
and time efficiently to contain costs and increase service value.
Promotes positive, supportive communication to all external
sources, patients, and staff. Strives for continuous improvement in
all aspects of the Physical Therapist function. Develops and
maintains a customer-oriented environment responsive to the needs
of all customers. Develops and revises plans of treatment and
discharge plans in conjunction with patients, families, and the
care delivery team. Provides an environment that promotes respect
for patients, their privacy, confidentiality, and property.
Initiates appropriate rehabilitative programs. Provides supervision
to ensure patient health, safety, and compliance with the plan of
care. Consults with and educates patients and families regarding
disease processes and self-care techniques. Implements home therapy
programs, including gait training, transfers, and activities of
daily living. Evaluates patient progress and adjusts physical
therapy programs as necessary. Instructs para-professional staff in
body mechanics and safe transfer techniques. Directs and supervises
the quality of patient care services provided by home health aides.
Interacts with patients and related parties to build positive
relationships. Ensures completion of all appropriate clinical
records needed for compliance with legislation. Maintains current
knowledge of available community-based services. Ensures patient
needs are met through the care delivery process. Makes constructive
recommendations for improving patient care. Participates in the
attainment of operational and financial objectives. Monitors
quality indicators and takes corrective action as needed. Develops
quality plans of care consistent with regulations and practices.
Demonstrates commitment to professional growth and competency.
Participates in education programs and shares acquired knowledge
