Job Description:
Summary: The Community Services Manager is responsible for the
overall operation of the Program including: planning, coordinating
and supervising services and staff to meet the individual needs of
each person served in a community based setting. Essential
Functions: Develop, write and implement person centered program
plans for people served. Interview, hire, train, supervise, monitor
and evaluate necessary program staff. Develop and communicate
routine scheduling of staff and people served. Maintain and submit
program time sheets and mileage sheets. Review billing and provide
feedback as necessary to ensure compliance and quality service
delivery. Provide coverage with participants in the program as
needed to meet program objectives and/or in the absence of program
staff Ensure compliance with OPWDD, Medicaid, DOH and any other
funders regulations by providing necessary training to staff,
implementing services and monitoring services. Complete incident
reporting process as required for Program. Develop opportunities
and ensure adequate support for people receiving services to
participate in recreational activities, develop life skills, make
meaningful choices, develop their abilities to problem solve and
decision make, and assist them in becoming contributing members of
the community. Participate in program planning meetings at least
semi-annually to coordinate services for people served. Implement
Life Plans (LP) as developed by each person served/family/advocate
and Care Coordinator. Complete person-centered planning activities
with people served as necessary. Complete monthly summaries as
required. Communicate with other Agency programs, families, outside
agencies and community resources as needed to coordinate services.
Maintain regular contact with people served to ensure a good
working knowledge of person’s needs, preferences and satisfaction
with services. Serve as a role model for people served and
represent the Agency positively in the community. Maintain thorough
documentation of services. Arrange and/or attend meetings and
trainings as required. Adhere to Agency policies and procedures.
Participate in corporate compliance and quality assurance
activities as required. Assist with Agency safety programs and
promotes workplace safety. Other related duties as assigned
Non-Essential Functions: Maintenance of Petty Cash account. Serve
on Committees and attend meetings as requested. Reporting

service at all times. Ability to instruct others on daily living,
social skills and quality programming. Ability to work as part of
an interdisciplinary team. Serve as a role model and represent the
agency positively in the community. Ability to self-initiate
projects and function autonomously as a leader when needed.
Physical Requirements: Ability to lift 50 pounds, and to stand,
sit, kneel, and stoop Ability to assist an adult with personal care
needs Be physically able to assist individuals in transferring from
wheelchair to other seat when needed Ability to assist individuals
with ambulation and to move chairs, tables and access storage
areas. Ability to use standard office equipment including personal
computer, phone, fax, copier etc. Ability to drive a vehicle.
Ability to speak, listen and sit. Working Conditions: General
program/ office setting and various community settings. Frequent
travel to other agency and community sites. May require use of
personal vehicle. Requires flexibility with work schedule to
include evenings and weekends Minimum Qualifications: Bachelor’s
Degree in relevant field with one year experience working with
people with disabilities or Associates Degree with 3 years’
experience working with people with disabilities High School
Diploma with five years experience working with people with
disabilities One year of supervisory experience is preferred Monday
- Friday
