We are currently expanding our team and seeking a dedicated Nurse Care Manager to coordinate and manage patient care with a personal touch. The ideal candidate will thrive in a flexible, dynamic environment and is passionate about providing individualized care to complex patients who have various health conditions. Assess patient health problems and needs, develop and implement nursing care plans, and maintain medical records. Administer nursing care to ill, injured, convalescent, or disabled patients. Advise patients on health maintenance and disease prevention or provide case management. Liaise with a multidisciplinary team to provide patient-centered care. Monitor, record, and report symptoms or changes in patients' conditions. Maintain accurate, detailed reports and records. Direct or supervise less-skilled nursing or healthcare personnel or supervise a particular unit. Consult and coordinate with healthcare team members to assess, plan, implement, or evaluate patient care plans. Strong ability to organize, prioritize, and multitask. Proficient in both verbal and written communication. Competence in providing evidence-based nursing care. Adept in building rapport with a diverse demographic of patients. Ability to handle emotional and stressful situations appropriately.
